- 13 exam rooms and one procedure room
- Consultation space for Care Management
- Diabetes educators and behavioral health providers
- Laboratory Services, including two phlebotomists, walk-in, no appointment, open to the community
- Radiology Services, including X-ray, echocardiogram, mammography and ultrasound, open to the community
